The standard SCAO packet includes FOC 65 (Motion Regarding Parenting Time), FOC 66 (Response), and FOC 67 (Order). Some counties add a scheduling order.

Where to File
File with the same circuit court Family Division that entered the current order. Pay the motion fee (typically $20 in Michigan) or request a fee waiver.
The Hearing
Friend of the Court usually sets a referee hearing first. Bring proof: school records, calendars, communications, and witnesses. Focus on how the change benefits the child, not on your dispute with the other parent.
